Most of the time, the patient is convinced they have torn a tendon or that surgery is inevitably coming next. Others believe it is \u201ejust a strain\u201d that will go away on its own.<\/p><p>The truth is that none of these conclusions is always correct.<\/p><p>Pain that occurs only when you raise your arm is a symptom, not a diagnosis. It allows us to raise our arm above our head, get dressed, drive a car, play sports, and perform hundreds of seemingly mundane gestures.<\/p><p>This freedom of movement, however, comes with a price.<\/p><p>Unlike the hip, where the femoral head is well contained within a deep socket, the humeral head is supported by a much smaller socket. Shoulder stability depends largely on the tendons, ligaments, and muscles surrounding it.<\/p><p>Among the most important structures are <strong>rotator cuff<\/strong>, a group formed by several muscles and their tendons. Their role is not only to move the arm, but also to keep the humeral head perfectly centered during every movement. This is explained by what happens inside the joint.<\/p><p>During arm elevation, the rotator cuff tendons and the subacromial bursa pass through a narrow space located beneath the acromion. If this space becomes smaller or if the tendons are already irritated, every movement can cause compression and pain.<\/p><p>That is why many people describe a very characteristic phenomenon: between approximately <strong>60 and 120 degrees<\/strong> of lifting pain appears, and after the arm passes this zone, the discomfort may even decrease. This phenomenon is known in the specialized literature as <strong>\u201epainful arc\u201d<\/strong> and represents an important clinical clue, although it is not specific to a single condition.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-78d90ca elementor-widget elementor-widget-heading\" data-id=\"78d90ca\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">It is not always a tendon tear\n<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-eaebb2c elementor-widget elementor-widget-text-editor\" data-id=\"eaebb2c\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>One of the biggest fears of patients is that any shoulder pain means a torn tendon. In reality, most cases have other causes.<\/p><p>Pain can occur in rotator cuff tendinopathy, subacromial bursitis, impingement syndrome, long head of the biceps tendinopathy, or even due to poor coordination between the scapula and the humerus.<\/p><p>There are, of course, partial or complete tendon tears as well, but these are only one possibility and must be confirmed by a clinical examination and, when necessary, by imaging studies.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-34f06d8 elementor-widget elementor-widget-heading\" data-id=\"34f06d8\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Sometimes the problem is not in the tendon, but in the way the shoulder moves\n<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-ec8627e elementor-widget elementor-widget-text-editor\" data-id=\"ec8627e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>This is an aspect that is still talked about too little. Raising the arm is not achieved solely by the shoulder joint. The scapula must rotate harmoniously on the thorax, and the muscles that control it must function synchronously. If this mechanism is disrupted \u2013 a phenomenon known as <strong>scapular dyskinesia<\/strong> \u2013 the stress on the tendons increases, even if they are initially healthy.<\/p><p>In other words, sometimes the tendon hurts not because it is \u201eweak,\u201d but because it is forced to work in an unfavorable biomechanical context. In reality, things are more nuanced.<\/p><p>In many situations, a careful clinical examination provides extremely valuable information. Musculoskeletal ultrasound, performed by an experienced physician, can dynamically evaluate the rotator cuff tendons, the bursa, and the biceps tendon, often being sufficient to guide the diagnosis.<\/p><p>An MRI is recommended especially when there is suspicion of a major tear, persistent symptoms that do not respond to treatment, or when the test result can change the therapeutic strategy.
